Player Registration Information

Returning Users:
Click on this Registration Link and then login into your existing Member Account. If you've forgotten your information, contact the appropriate Registrar.

New Users:
Click on this Registration Link and then create a Member Account, completing all requested data. Once completed, a confirmation email will be sent to you, with the player registration form as an attachment. Print the form, sign it, and give it to your coach.

If your child is new to FWSA, a copy of his/her proof of age document, i.e. birth certificate, passport, etc., will be required. They cannot play until this is accomplished. If the player isn't currently associated with a team a Coordinator will contact you once you've registered on-line. Be sure to retain the player registration form for future team placement.

Notes:
1. Players must register under their legal age bracket, even if 'playing up'.
2. If unable to find the automated Player Registration Form, please complete and sign the FWSA Medical Release Form below and give to your coach.

Mod 2011 Spring Registration
This spring the Federal Way Mod Club will offer a spring soccer league for U6 - U10 players. There will be 8 games with most games being played on Sunday and a few on Saturday from April 17 – June 12, 2011. Teams will practice at least once a week.

Each player will receive a FWMSC tee-shirt and parents/guardians will need to provide shorts, socks, shin-guards and appropriate shoes.

Registration Procedure

1. Register Online between January 1 - March 1, 2011.       

2. Attend one registration date below to complete the registration process.  You must attend a registration date for your player to be placed on a team.

3. Bring the following with you to the registration date:

• Online registration confirmation – after you have registered online you will receive a registration confirmation in your email.  Please print the attachment and bring with you to registration.  If you do not receive a confirmation please email Novella Fraser at fwregistrar@yahoo.com
• Birth certificate – if your player hasn’t played for Federal Way Mod in the past bring a copy of the player’s birth certificate.  We will not keep the birth certificate.
• Check or money order - made payable to FWMSC.  Scholarship request are also available and scholarship form is located below.

Grants/Scholarships

While the Clubs rely on player fees and fundraisers to pay for their expenses, in the event a player cannot afford the Club registration fees, assistance may be available to those who qualify; Participation Grants for FWUSC players, Scholarships for FWMSC players.

Mod Scholarship Request Form - Any family requesting a scholarship for their child's player registration fees, must fill out this form and turn it into the Club at the time of registration.

Referee Clinic Reimbursement

Mod Reimbursement - The Federal Way Mod Soccer Club (FWMSC) will cover instruction and test fee for all players, parents, and coaches of FWMSC teams. For more information please click on Mod Referee Clinic Reimbursement Instructions for more information.

Staff Registration
You can register as a team or personnel for a Federal Way Soccer Team. Anyone working with our youth players or handling team funds must register online through the Bonzi system and be Risk Management Approved (RMA). This includes, but is not limited to, a Coach, Assistant Coach, Trainer, Manager, and Treasurer. An electronic confirmation of your registration will be sent to you.

Coaching Application
In addition to registering online, all Federal Way coaches and prospective trainers are required to submit a Coaching Application.

This code of ethics has been developed to clarify and distinguish approved and accepted professional, ethical and moral behavior from that which is detrimental to the development of soccer within the Washington State Youth Soccer Association (WSYSA).

Team Registration

Mod Team Registration
No Team Registration Form is required this spring.

Code of Ethics
The Coaches' Code of Ethics form should be signed by every head coach indicating that they have read and will abide by the WSYSA Code of Ethics. This signed form is to be turned in with their team registration packets. The Player's Code of Ethics and the Parent's Pledge to be a Good Sport are tools for the coach, if desired.
